How to tell if a bug bite is infected
核心要点 / 快速概览

Most bug bites are red, swollen, and itchy, but persistent scratching can introduce bacteria into the skin. Here is how to tell if an ordinary bite has turned into a bacterial infection like cellulitis.

Every bug bite causes local inflammation — redness, swelling, and itchiness are normal immune responses to insect saliva or venom. However, when you scratch a bite, your fingernails create tiny breaks in the skin barrier. This allows bacteria (typically Staphylococcus or Streptococcus) to enter and multiply, leading to secondary bacterial infections such as impetigo or cellulitis.

1. Normal reaction vs. bacterial infection

Timing and progression are the most reliable indicators. A normal bite reaction peaks within 24 to 48 hours and gradually improves. An infection typically develops 2 to 5 days after the bite, often after intense scratching, and gets progressively worse rather than better.

  • Normal bite: Redness is contained, swelling stabilizes after 24–48 hours, skin is itchy rather than throbbing, clear fluid may form a small blister.
  • Infected bite: Redness spreads outward after day 2, skin feels hot and tender to touch, pain increases with throbbing, cloudy pus or yellow crusting appears.

2. Red flag: Red streaks spreading from the bite

If you notice red lines or streaks tracking along your skin away from the bite toward your heart, this is a sign of lymphangitis (infection traveling through the lymphatic vessels). This requires prompt medical evaluation and prescription antibiotics.

Tip: Use a pen to draw a light circle around the outer edge of the redness. If the red border expands significantly past the pen line over the next 6 to 12 hours, seek medical attention.

3. Safe first aid and infection prevention

  • Wash immediately: Clean the bite with mild soap and cool water.
  • Do not scratch: Keep fingernails clean and short; use an ice pack wrapped in cloth to numb the itch.
  • Apply an over-the-counter antiseptic or hydrocortisone cream to reduce itching.
  • Keep it covered: A breathable adhesive bandage prevents unconscious scratching while asleep.

When to see a doctor

Bacterial skin infections do not respond to antihistamines or anti-itch creams — they require prescription topical or oral antibiotics. Seek urgent medical care if you develop fever, chills, dizziness, expanding redness larger than your palm, or dark blisters.

常见问题

Can a bug bite get infected days later?

Yes. Most secondary infections occur 2 to 5 days after the bite because scratching breaks the skin and allows bacteria from your hands and nails to enter.

What does pus in a bug bite mean?

Clear serum in a mosquito or blister is normal. Opaque, milky, yellow, or foul-smelling pus indicates a bacterial infection that should be evaluated by a healthcare professional.

How can I tell cellulitis from a normal large bite?

Cellulitis causes increasing skin warmth, expanding tight red skin, throbbing pain, and often fever. Normal bites peak by day 2 and itch more than they hurt.
